Submit by December 15 to earn your $0 premium beginning January 1
Complete a dental, vision, and medical wellness visit and submit your VoW form to earn premium rewards for 2022. If you submit your VoW later, your $0 premium will take effect the month after you submit it (see due dates below).
If you have completed your VoW at any time throughout the 2021 plan year, your $0 premium benefit will also apply throughout 2022. However, if any of your wellness visits occurred in 2020, you will need to complete a new VoW to be eligible for the $0 premium in 2022.
For example, if you have your vision signature from 2020, but health and dental from 2021, you will need a new signature from your Optometrist to qualify for a $0 premium in 2022. Additionally, you will need to resubmit all three signatures.
Please note, the VoW visits and the IncentiCare Health Screening are separate events. While completing the VoW will get you free medical insurance (ie: a $0 premium), your Health Screening determines your deductible.
