Prior Authorizations

Prior Authorization is required for:

  • Inpatient admissions
  • Inpatient/Outpatient surgeries
  • Partial Hospitalization and Intensive Outpatient for Mental Health/Substance Abuse
  • All services listed here regardless of place of service.

All services are subject to medical necessity.

Prior authorizations are submitted to and reviewed by Cigna. Before calling with a prior authorization request, you must verify benefits and coverage by clicking on "Check Patient Eligibility" below
